Does the DM have to contain a link at all?

No. A campaign can send a message with no link, which suits a reply that is only meant to start a conversation. When there is a link, it is tracked, and one campaign carries one link.

What happens to comments Instagram never sends a webhook for?

A polling sweep re-reads recent comments on active campaigns every few minutes and picks up anything the webhook missed. It skips comments that already have a delivery logged, so nobody is messaged twice.
